Before you begin

You do not need to have everything. Once you tell us what kind of product it is, the form works out exactly which documents apply to it and shows you the list. No upload is required to send the form. Attach what you have, tick off what you do not, and send it either way — most gaps are things your manufacturer can email you the same day.
GMP
No ISO 22716 certificate? Not a blocker. Where the manufacturing site is not certified, a signed declaration from your manufacturer carries the same weight in your file. You will find our declaration template ready to download in the document checklist, under Manufacturing standards certificate.

What is useful to have open

None of these are required to submit. They just save you a second round of emails.

Your recipeEvery ingredient and its exact percentage. You type it straight into the form — no spreadsheet or template needed.

Your labelThe artwork file, or just a clear photo of the back of the pack.

Supplier paperworkSafety data sheets and quality certificates for your ingredients. Your manufacturer holds these.

Test reportsStability, preservative or skin tests, if any have been done.

One thing we cannot work around. If you type your recipe in, the percentages have to add up to exactly 100%. The safety calculations are built on them. The table adds up as you type, so you will see straight away if something is off — and if you would rather upload the sheet from your manufacturer, do that instead.

Nothing saves by itself. If you need to stop, press Save draft at the bottom of any page. It downloads a small file you can load back in later. Closing the tab without it loses your answers.

Is my recipe safe with you?

Confidentiality

Yes. Your formulation is commercially confidential. It is used only to prepare the documents you ask for, held under UK GDPR, and never passed to anyone else without your written say-so. We keep records for ten years because a Responsible Person is required to. If you would rather have an NDA signed first, email compliance@phoenixcosmeticsafety.com before you start.
